 I, the model, grant you, the photographer, the full right of image usage of the photographs subject to the following conditions:

I understand that I grant to Faisal Photography Studio and  Faisal Al Shaqsi ("Photographer") the absolute and irrevocable right and unrestricted permission concerning any photographs that he/she has taken or may take of me or in which I may be included with others, to use, reuse, publish, and republish the photographs in whole or in part, individually or in connection with other material, in any and all media now or hereafter known, not limited to the internet, and for any purpose whatsoever, specifically including illustration, promotion, art, editorial, advertising, and trade,Public display without restriction as to alteration.

As this is a commission paid work i understand i do not have the right to ask for any photo taken in the session. Unless offered by the studio or photographer.

The photographs may be used to represent an imaginary person and any wording associated will not be attributed to me unless my name is specifically mentioned..

 

I release and discharge Photographer from any and all claims and demands that may arise out of or in connection with the use of the photographs, including without limitation any and all claims for libel or violation of any right of publicity or privacy.

 

This authorization and release shall also inure to the benefit of the heirs, legal representatives, licensees, and assigns of Photographer, as well as the person(s) for whom he/she took the photographs.

I acknowledge that by signing this form, subject to the usage restrictions above, I give up all claim of ownership of the photographs, and assign copyright to the photographer named above. No further payment will be due. Use of the photographs may be granted to third parties, however the photographs will remain the property of the photographer.

Health & Safety

I understand that health and safety is my sole responsibility and that i should be careful during the photo shoot and inform photographer or studio owner prior to the shoot of any illness or injury or allergy to specific production no limited to makeup that may cause me harm and any pose that can't be done due to an injury.

 

I understand that Faisal Photography Studio and members wont be held liable for any injury or health issues caused during the photo shoot or allergy caused not limited to makeup, accessories, dress and food.

I am a legally competent adult and have the right to contract in my own name. I have read this document and fully understand its contents. This release shall be binding upon me and my heirs, legal representatives, and assigns.
